Tri-Valley, Inc. provides the following in-home services and programs for elders age 60 and over and younger people with disabilities. Our Information and Resource (I & R) specialists will help identify which services are best suited for you.
Adult Family Care
provides family living as an alternative to institutional care. Participants are carefully screened and matched with caregivers who provide 24-hour support in their homes. In some cases, family members can be paid for caregiving.
Consumer Directed Care
allows consumers to hire and manage the delivery of their homemaking and personal care services.
Home Care/Care Management
provides in-home services to elders age 60 and over who need assistance so they may continue to live at home. Care Managers first complete in-home assessments and then develop and manage care plans for elders that promote independent living.
Money Management
matches trained, insured and supervised volunteers with elders who need help balancing the checking account, setting up a budget, writing checks and sorting mail.
Nursing/Clinical Assessment & Eligibility
Registered Nurses perform screenings to determine the elder’s clinical and medical eligibility to enter a skilled nursing home or attend Adult Day Health, plus in-home assessments to determine the need for personal care.
Personal Care Attendant
allows consumers to select, train and employ their own caregivers with support from social work and nursing staff.
Protective Services/Crisis Intervention
investigates and resolves allegations of elder abuse including physical, emotional, sexual, financial exploitation, neglect or self neglect.
