Tri-Valley provides coordination of the Supportive Housing Site located at Howe Village in Spencer; Central Park Terrace and Winter Street Apartments in Franklin; Birmingham Court, Fairfield Court, and Greenleaf Terrace in Milford; and Sunrise Apartments in Mendon. It is an independent living model with built in supports for all residents. Tri-Valley works closely with Housing Authorities and the direct care vendors at each supportive site. Tri-Valley provides a Resident Services Coordinator at each supportive housing site and direct care vendors to meet residents’ needs.
The Tri-Valley Coordinators assist with connecting residents with community services, in-home care and information as well as assisting the housing director in coordinating educational and social events for the residents.
